Insurance and Fees

I am accepting new patients using private pay, out of network coverage, or Aetna insurance. For out of network a super bill can be provided to submit to your insurance for reimbursement.

Good Faith Estimate/No Surprises Act
​
If you are not using or do not have insurance you are entitled to a Good Faith Estimate. This estimates the costs of non-emergency health care. You will receive a Good Faith Estimate in writing at least one business day before your appointment. If the final bill exceeds $400 or greater than the initial estimate you have the right to dispute the charge. For more info on your rights visit: www.cms.gov/no surprises or call 1-800-985-3059.

Deductibles and Payments

Most insurances have out of pocket deductibles that renew each fiscal year. If the deductible is not yet met (paid in full) the patient may be billed for the full cost of the visit until the deductible is met. Contact your insurance carrier to better understand your deductible.

Out of Network Coverage
​
Many insurances plans offer partial reimbursement for "out of network" visits. Calling your insurance is the best way to fully understand these benefits.

Any costs not covered by insurance are the patient's responsibility. Full payment is due at the time of service unless you are using an in network insurance.
