Insurance and Fees
I am currently accepting new patients using in and out of network insurance or private pay.
Please contact me for an out of pocket fee schedule.

Prior to your initial appointment please contact your insurance to verify in network coverage with Mary "Katie" Farnsworth, PMHNP specifically with Firn Line Wellness.
Note in addition to my private practice I work for multiple hospital systems as both a midwife and a psychiatric NP so it is essential to verify that your insurance is in network with me at Firn Line Wellness as coverage will not be provided if I am only listed as an in network provider elsewhere.
Out of Network Coverage
Most private insurances plans offer varying reimbursement rates for "out of network" visits.
Firn can supply a "superbill" for you to submit for reiumbursement.
Calling your insurance is the best way to fully understand these benefits.
Any costs not covered by insurance are the patient's responsibility. Full payment is due at the time of service unless you are using an in network insurance.
Deductibles and Payments
Most insurances have out of pocket deductibles that renew each fiscal year. If the deductible is not yet met (paid in full) the patient may be billed for the full cost of the visit until the deductible is met. Contact your insurance carrier to best understand your deductible and copays for psychiatric services.
Good Faith Estimate/No Surprises Act
If you are not using or do not have insurance you are entitled to a Good Faith Estimate. This estimates the costs of non-emergency health care. You will receive a Good Faith Estimate in writing at least one business day before your appointment. If the final bill exceeds $400 or greater than the initial estimate you have the right to dispute the charge. For more info on your rights visit: www.cms.gov/no surprises or call 1-800-985-3059.
